Case Manager - Day Shift

This is a 13-week contract position.

Shift: 5x8 hour days (08:30 - 17:00)

The Case Manager is responsible for conducting medical necessity reviews, discharge planning, and utilization reviews. This role involves identifying and communicating changes in the patient's plan of care to the healthcare team. The Case Manager maintains a comprehensive knowledge of community resources and the referral process, utilizing Length of Stay (LOS) data to anticipate patient needs, manage resources efficiently, and ensure timely transitions to alternative levels of care. Documentation in the patient record must be completed in a consistent and timely manner, including initial screenings and discharge plans.

• Conduct medical necessity reviews and assessments.
• Develop and implement discharge plans in coordination with the healthcare team.
• Identify changes in patient care plans and communicate promptly.
• Leverage community resources effectively to meet patient needs.
• Document patient interactions and care plans accurately.

• Minimum of 2 years of case management experience is preferred.
• Experience with Epic charting system is required.
• Charge experience is preferred.
• Prior experience in community hospital settings is preferred.
• State or Compact license must be in hand at the time of submittal.
