About This Role The Registered Nurse (RN) in Ambulatory Internal Medicine is responsible for providing age-specific care to a designated group of patients. This role is ideal for nurses who excel in collaboration and patient education while utilizing their clinical judgment. Highlights Location: Yorktown Heights, NY Schedule: Full-Time, Day shift What You'll Do Assess the patient’s physiologic health status through interviews and comprehensive health assessments. Evaluate the patient’s psychosocial health status and identify cultural/ethnic requirements. Develop criteria for measurement of patient outcomes and establish individualized patient goals. Formulate and document the plan of care, coordinating with physicians and health team members. Implement the plan of care, ensuring safety precautions and effective communication with patients and families. Evaluate care provided for patient outcomes and perform variance analysis for performance improvement. Collaborate with care team members and provide direction in executing treatment regimens. Provide wound care and manage pressure ulcers (Stages I–IV). Administer medications correctly and educate patients on drug interactions. Participate in patient and family education, coordinating referrals for follow-up care. Requirements Education: Graduate from an accredited School of Nursing Bachelor’s Degree in Nursing preferred; must enroll in an accredited BSN program within two years and obtain a BSN within five years of job entry date Experience: Experience in a nursing role preferred Licenses & Certifications: Current license to practice as a Registered Professional Nurse in New York State required Specialized certifications as needed About Northwell Health Northwell Health is a leading healthcare provider committed to delivering exceptional patient care.
