Physical Therapist Assistant (PTA) Registry Physical Therapist (PT) Position Overview Provides quality rehabilitation care in Physical Therapy as outlined in the plan of care through therapeutic exercise, massage, and other treatments in accordance with laws, regulations and facility guidelines. Essential Job Duties Assess functional levels and capabilities of residents. Conduct ongoing assessments of resident muscle strength, range of motion, coordination and balance and make recommendations related to resident therapy treatments. Provide physical therapy treatments by following the resident plan of care. Develop measurable and realistic goals based on resident’s care plan. Participate in the discharge planning process for residents. Thoroughly document treatment progress in resident charts accurately and timely. Maintain a positive working relationship with residents and families. Document appropriately and timely in accordance with all laws, regulations and facility guidelines. Perform overall supervision to PTAs and rehabilitation aides to ensure quality rehab care services are provided. Complete required facility forms and documents to ensure compliance with state and federal regulations. Other Job Duties Attend and participate in meetings, conferences and seminars. Provide in‑service education to facility staff as it relates to rehab services. Attend meetings and communicate information in a productive fashion. Assist with transporting residents from room to therapy gym, etc. Provide training and education to family, residents and staff for physical therapy and use of assistance devices and equipment. Conduct home evaluations prior to discharge to ensure safety of residents. Schedule appointments, take calls, reorder supplies and complete insurance forms. Schedule 8‑hour shift Holidays Monday to Friday Weekend availability Work Location One location Work Remotely No Qualifications Education & Certification: Graduate of an accredited physical therapy program (BS or MS‑PT). Skills & Requirements: Ability to read, write and speak English; communicate resident condition to others; make independent decisions; patient, friendly, courteous and respectful to residents; implement physical therapy programs and goals; move frequently throughout the workday; handle sensitive situations with patience, dignity and respect; lift 40 lbs floor to waist, 10 lbs waist to shoulder, carry 40 lbs, and push/pull 40 lbs. Experience: Current PT license in state of practice in good standing; one year experience in a healthcare setting, long‑term care experience preferred.
