Position Title: Advocate Level 2 (Care Advocate II )
Department: Care Management – Enhanced Care Management (ECM) & Community Supports
Reports To: Care Management Supervisor
Supervisory Chain: Care Management Supervisor → Director of Care Management
Work Location: Office-Based with Community Fieldwork Requirements

Summary

The Advocate Level 2 (Care Advocate II) is a frontline position responsible for delivering Enhanced Care Management (ECM) and Community Supports (CS) services to Medi-Cal members experiencing complex medical, behavioral, housing, and social needs. This position supports individuals in overcoming barriers related to Social Determinants of Health (SDOH) through person-centered engagement, care coordination, advocacy, and linkage to essential resources and services.

Advocate Level 2 staff are proficient at the core services delivered through Enhanced Case Management (ECM02), Housing Transition Navigation Services (CS02) and Housing Tenancy and Sustaining Services (CS01), under the California Advancing and Innovating Medi-Cal (CalAIM) initiative. Services are delivered in alignment with requirements established by the California Department of Health Care Services (DHCS) and monitored through the Central Coast Alliance for Health (CCAH).

Advocate Level 2 staff have an competencies that offer them more enhanced levels of service provision towards vulnerable populations including those experiencing challenges in mental health, physical health, psychosocial functioning, oppression, and those that are justice involved. Advocate Level 2 have an aptitude and history of maintaining caseloads of at least 35 members and at times more than 40 members as well as being able to transition between service modalities beyond ECM and CS. Advocate Level 2 have the ability to provide some supervisory guidance and/or support to other Advocate 1 and/or other staff.

EPIC has established a classification and job promotion hierarchy in which Advocate Level 1 can request reclassification to be level 2 as they near their first year of employment and have finished all necessary milestones, have quality performance reviews, and have completed the necessary trainings. Advocate Level 2 are the standard placement for new advocates unless they have adequate history providing services or have specific degrees like MSW.

In addition to direct member support, Advocate Level 2 staff contribute to agency-wide operations and may support outreach, community engagement, enrollment efforts, administrative functions, and other operational activities necessary to advance the mission and functioning of EPIC, provided such duties are consistent with the employee’s training, abilities, and role expectations.

Duties and Responsibilities

Advanced Care Coordination

• Independently manage a full caseload of members
• Conduct comprehensive assessments and develop care plans
• Lead coordination with healthcare providers, behavioral health, and community partners
• Provide ongoing case management and follow-up services

Community Supports (CS01 & CS02)

• Lead housing transition navigation and tenancy support services
• Resolve complex housing barriers and service coordination challenges
• Build relationships with landlords, housing providers, and community agencies
• Support program efficacy by engaging in leadership opportunities

Documentation & Compliance

• Ensure high-quality, timely documentation in PEAR Suite
• Maintain compliance with CalAIM, CCAH, and billing requirements
• Support audits, chart reviews, and corrective action processes
• Ensure claims readiness within required submission timelines
• Provide auxiliary support to other advocates when needed

Leadership & Support

• Provide informal mentorship and guidance to Advocate Level 1 staff
• Assist supervisors with training and onboarding support
• Contribute to program improvement and workflow development

Productivity & Performance

• Meet or exceed agency productivity standards
• Demonstrate consistent achievement of engagement and service benchmarks
• Support agency goals related to quality care and member outcomes

Supervisory Responsibilities

Advocate Level 2 provide some supervisory guidance and peer support to other advocates and in particular Advocate level 2. While Advocate Level 2 are not directly providing management or supervision to employees – they have shown an aptitude to be of support to management structure at EPIC.

Minimum Qualifications

• High school diploma or equivalent required.
• Educational degree, social service certification, or professional career experience as a direct service provider to vulnerable populations
• Minimum 1-2 years of experience in case management, ECM, or similar services
• Strong knowledge of community resources and healthcare systems
• Demonstrated ability to manage complex cases independently

Preferred Qualifications

• Master’s degree in social work, psychology, sociology, public health, human services, or related field.
• Multiple years of experience working within Medi-Cal, CalAIM, ECM, Community Supports, healthcare, housing, or nonprofit settings.
• Proficient understanding of Monterey County community resources and social service systems.
• Bilingual abilities strongly preferred depending on community need.

Physical Demands and Work Environment

This position is primarily office-based and requires regular in-office attendance. Field-based responsibilities are also required to support member engagement, home visits, appointments, housing navigation, and community outreach activities.

Work may occur in office environments, healthcare settings, shelters, housing environments, and community locations.

• Ability to travel locally for field-based services and community engagement.
• Ability to sit, stand, walk, bend, and lift light materials consistent with office and field responsibilities.
• Reliable transportation, valid driver’s license, and proof of insurance are required depending on assigned duties.
