Physical Therapist - Home Health

Location: Gassaway, WV

Schedule: Day shift, 8:00 AM - 4:30 PM, Monday - Friday

Assignment Duration: This is a 13-week contract position.

Job Description: Join our home health team providing exceptional care to patients in their homes. You will treat a diverse patient population with common diagnoses including general orthopedic conditions, generalized weakness, strengthening needs, and cardiac rehabilitation.

Responsibilities:

• Conduct evaluations and develop treatment plans for patients.
• Implement treatment protocols in accordance with patient needs.
• Document patient care and progress using the Epic documentation system.
• Communicate effectively with patients, families, and healthcare team members.

Required Experience / Certifications / Licensure:

• BLS Certification
• Valid Physical Therapy license for the state of West Virginia
• Experience in home health preferred but not required.
