Physical Therapist (PT) – Home Health

Contract | Days | No weekend or holiday requirements | Self-scheduling available

Key Responsibilities

• Provide physical therapy services to patients in a home health setting across an assigned multi-county coverage area
• Manage a caseload of approximately 5–6 units per day, including start of care, discharge, recertification, and regular visits
• Treat patients with diagnoses including general orthopedic conditions (hips, knees), generalized weakness, strengthening needs, and cardiac rehab
• Complete clinical documentation accurately and in a timely manner using the Epic system
• Coordinate scheduling and patient visits according to geographic area while adhering to initial and reassessment guidelines

Required Qualifications

• Active Physical Therapist licensure meeting all requirements for practice in the state of West Virginia
• Current Basic Life Support (BLS) certification
• Experience performing standard physical therapy evaluations and treatments in a clinical or home health setting
• Ability to travel across an assigned multi-county service area

Preferred Experience / Skills

• Prior home health experience
• Familiarity with or willingness to learn Epic electronic medical records documentation
• Candidates able to cover the Webster and/or Nicholas County areas are strongly preferred
