Our staff is known for its integrity and commitment to their roles, which creates a supportive and productive work culture.Key ResponsibilitiesConduct thorough evaluations of patients, including reviewing relevant medical histories and performing specific assessments to determine personalized treatment plans.Communicate evaluation findings and treatment options clearly to patients and their families, actively involving them in goal-setting to ensure a patient-centered approach.Implement evidence-based treatment protocols tailored to each patient's needs, continually adapting our strategies as their conditions evolve.Document changes in patient conditions meticulously and consult with other healthcare professionals as necessary, ensuring coordinated care and timely modifications to treatment plans.Oversee discharge planning and provide follow-up care recommendations, ensuring seamless transitions and continuity of care.Demonstrate effective time management by maintaining a well-organized schedule, attending meetings punctually, and participating in marketing initiatives to promote our services.Engage in continuous professional development, including pursuing further education and serving as a clinical instructor to students.Contribute to overcoming barriers to care by understanding access measures and participating in initiatives aimed at improving service delivery.Patient Population / Client FocusOur physical therapist will primarily serve a diverse patient population, including individuals recovering from surgery, injury, or chronic conditions. The focus is on delivering compassionate, tailored rehabilitation services that promote independence and enhance quality of life. Our therapists strive to establish trust and rapport with both patients and their families, ensuring that care is holistic and respectful of each individual's preferences.QualificationsA Doctorate degree in Physical Therapy (DPT) from an accredited program.A valid state license to practice physical therapy in Florida.Current American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers.Strong communication skills and a commitment to patient-centered care.Preferred ExperienceAt least 2 years of experience in a clinical setting, preferably in home healthcare or outpatient rehabilitation.Compensation and BenefitsThe competitive annual salary for this position ranges between $100,000 and $120,000, commensurate with experience and qualifications.
