Physical Therapist (PT) – Home Health

Location: Seaside, OR

Shift: Day Shift | 8-Hour Days

Job Summary

The Home Health Physical Therapist (PT) provides individualized rehabilitation services to patients in their homes, helping them improve mobility, strength, balance, functional independence, and overall quality of life. The PT develops and implements treatment plans, performs evaluations, and collaborates with physicians and interdisciplinary healthcare teams to achieve optimal patient outcomes.

Required Qualifications

• Graduate of an accredited Physical Therapy program.

• Active Oregon Physical Therapist License or eligibility to obtain.

• Current BLS Certification.

• Minimum 1 year of Skilled Home Health experience required.

• OASIS experience required.

• Valid driver's license and reliable transportation.

• Strong assessment, documentation, and patient education skills.

• Ability to work independently in a home health environment.

Responsibilities

• Perform comprehensive patient evaluations in home settings.

• Develop and implement individualized treatment plans.

• Provide therapeutic exercises, gait training, balance training, and mobility interventions.

• Complete OASIS assessments and documentation.

• Educate patients and caregivers regarding treatment plans and home exercise programs.

• Monitor patient progress and adjust treatment plans as necessary.

• Collaborate with physicians, nurses, and interdisciplinary care teams.

• Maintain accurate and timely documentation within EMR systems.

• Ensure compliance with home health regulations and agency policies.
