Director of Rehabilitation (DOR)
Lead Two Programs. Transform Lives Across Two Communities. Build Something That Lasts in Oakland.
Oakland Heights Nursing and Rehabilitation and Lake Park Healthcare Center are seeking an experienced Director of Rehabilitation to provide unified therapy leadership across both facilities in Oakland, California. This is a rare and meaningful dual-facility leadership opportunity for a licensed therapy professional ready to take full ownership of two thriving rehabilitation programs in a collaborative, mission-driven organization.
Oakland Heights Nursing and Rehabilitation is a 48-bed skilled nursing community. Lake Park Healthcare Center is a 35-bed skilled nursing community. Together they represent a combined 83-bed clinical portfolio with the scale, patient volume, and operational complexity that experienced DORs with genuine leadership depth will find both challenging and rewarding.
Our Mission: We are professionals committed to serving others, nurturing life, and promoting an individual's ability to thrive.
Why This Opportunity Is Different
Leading two facilities under a single employer provides clinical variety, organizational influence, and career development scope that a single-site DOR role simply cannot match. You will shape therapy culture, staff performance, and patient outcomes across two distinct communities while building the kind of regional leadership experience that positions you for continued advancement within a growing multi-site organization.

- Clinical autonomy to lead, build, and shape therapy services across Physical Therapy, Occupational Therapy, and Speech-Language Pathology disciplines at both facilities
- A collaborative, mission-driven organization that genuinely invests in its clinical leaders
- Dual-facility leadership experience that strengthens your regional and multi-site career trajectory
- A stable, professionally managed environment aligned with federal, state, and local care standards
