Impactful Adult Mental Health Case Manager

Northwestern Community Services Board

Northwestern Community Services Board seeks a dedicated professional to manage an assigned caseload of adults with mental health needs. You will assess client needs, develop individualized service plans, and coordinate with community resources to achieve service goals. The role requires QMHP eligibility, a Bachelor’s degree in human services, and at least one year of experience with adults.
