Compensation: 43k - 47k - salary is dependent on experience.

Hours: Full-time/Monday-Friday, 9am-5pm
Day to Day
This position is responsible for intensive strength-based case management support of youth entering the YES Housing, Rapid Rehousing and After Care Programs. The Housing Case Manager focuses on ending the cycle of homelessness through education, programs, and resources that best meet the individual needs of homeless and at-risk youth.
Responsibilities:
- Encourage a team approach by cooperative interaction and communication with colleagues, volunteers, and community agencies.
- Assessing the physical needs of youth and meeting basic needs as appropriate, including, but not limited to, first aid, food, hygiene products, community resources, clothing items, etc.
- Conduct comprehensive client assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information as appropriate to develop and implement a service plan designed to promote independent living.
- Conduct regular home visits to clients to assess needs, provide support, and ensure continuity of
care in a safe and respectful manner. - Develop support systems to meet client needs by identifying and coordinating a variety of available services.
- Conduct in-home and on-site visits, housing inspections and curfew checks.
- Determine the quality and effectiveness of services provided and if necessary, assist with the development and implementation of new processes and procedures.
- Engage high-risk (trafficking, DV) youth and connect to appropriate services.
- Maintain confidential information concerning the agency, clients, and program organizational plans, policies, and strategies.
- Maintain accurate and detailed record keeping and documentation according to agency requirements.
- Advocate for youth in appropriate life arenas including, but not limited to, interactions with family, law enforcement agencies, courts, and schools and by providing referrals for access to appropriate services.
- Demonstrate the ability to use appropriate crisis intervention and problem-solving skills in providing basic services to youth.
- Assist clients in achieving goals towards independence.
- Attend YES staff meetings and required training as assigned
Skills/Competencies/Requirements:
- Knowledge of, and ability to collaborate with, agencies and organizations addressing the issues of at-risk youth.
- Knowledge of child/adolescent development and pertinent issues affecting homeless youth and/or families.
- Acceptance and respect for diverse populations.
- Ability to develop and maintain working relationships with youth and their families, other staff, professionals, community members, and law enforcement agencies.
- Ability to set and maintain professional boundaries with clients.
- Regular and predictable attendance is required.
- Must be available for occasional weekend or late evening shifts.
- Must be available to commit to one week a month on-call rotation.
- Must share in the agency's value of cultural competency and a willingness to be trained further in the understanding of different cultures.
- Ability to work with minimal supervision.
- Valid motor vehicle operator’s license, current auto insurance, and ability to transport program youth as needed.
- Completion of thorough criminal history and background investigation.
Education:
- Bachelor’s Degree from an accredited college or university in human services field such as social work, psychology, counseling, sociology, or a related field.
- Minimum two years of social or human service experience is preferred.
- Knowledge of Positive Youth Development (PYD) factors.
- Experience with youth homeless systems is a plus.
- Case Management experience is preferred.
