To implement and deliver all aspects of peer support and to assist program participants in achieving short‑ and long‑term goals.” Salary: $73,000- 77,000 per year Location onsite 5 days a week Responsibilities The Program Manager will be responsible for overall ADSP Program Staff Supervision, advocacy and support services to eligible families and individuals for a caseload of 12‑15 families/individuals. The Program Coordinator also helps people access other applicable housing opportunities and assists clients in maintaining or increasing their housing stabilization. Working to assess the participants' needs, by providing immediate services, personal goals, and service plans. This position will work with participants around self‑sufficiency, and how to maintain independence and prevent homelessness. Through the program, clients may receive case management referrals to community support and counseling. The Program Manager provides advocacy and referral services as needed to enrolled clients, and where possible, assist with maintaining housing. Program Intake, Enrollment, and Application Management Serve as the primary point of contact for new applications and program enrollments submitted through the DATAVERSE system by partner agencies. Receive, review, and process new applications for subsidy enrollments. Review case management applications and supporting documentation to ensure they are complete, accurate, and in compliance with Catholic Charities and MOHCD/Subsidy Program policy and procedure prior to enrollment. Create and manage new client enrollments in the program roster, CARES (internal client management system), and the GMS system (funding system). Divide new applications into case managers in coordination with the Senior Program Director. Case Management Support & Client Services Provide comprehensive screening and assessment of clients to determine service needs. In collaboration with the Senior Program Director, provide consultation and guidance to case managers regarding application processing, policy clarification, and client inquiries. Provide ongoing client support, including information and referrals, regular client contact, and assistance in achieving service plan goals. Arrange and coordinate services to address clients’ immediate and long‑term needs as documented in individualized care plans. Maintain a minimum of monthly contact with clients to support housing stability and life management skills. Develop and maintain person‑centered care plans, including counseling, monitoring progress, and modifying plans as needed. Coordinate housing stabilization plans for assigned families/clients, including: Share other subsidizing housing opportunities and assist the client with the applications. Obtaining and reviewing required landlord documentation. Making accurate and timely referrals to internal and external service providers. Mediating with landlords, property managers, attorneys, or legal representatives to obtain required documentation and prevent eviction. Requesting payments for additional assistance such as back rent, furniture and basic needs. Update program rosters (ADSP and OALTS) to reflect approved changes, additional payments, and client status updates. Generate monthly reports for Accounts Payable to issue subsidy payments (ADSP and OALTS). Process and track monthly check requests for payments outside of subsidies (e.g., basic needs, back rent, first month’s rent, security deposits) for both programs (ADSP and OALTS). Coordinate‑Schedule and deliver special handling checks on the first business day of the month. Compile required funder monthly reports through the GMS system. Policy, Procedures & Program Administration Assist the Senior Program Director in creating, drafting, and updating program policies, procedures, and forms as required. Draft, adapt, and maintain version control for the Anti‑Displacement Subsidy Program policies and procedures. Train new staff and partner agency personnel in program policies, procedures, and systems. Provide ongoing policy and procedural support to program case managers. Serve as the focal point for external agency representatives during referrals. Support the Senior Program Director in organizing and processing data for the OALTS Program. Data Management, Systems & Internal Coordination Update client records, service authorizations, and reports in CARES. Develop templates, presentations, and materials for internal use and for partner agencies. Coordinate with partner agencies and other Catholic Charities programs and community partners to ensure effective communication and high‑quality service delivery. Education & Experience BA degree and one year of experience preferred or five years related experience. Two years of experience providing client case management services with the housing insecure population and/or service programs preferred. Experience working with families in crisis. Experience with San Francisco housing resources and public benefits. Other relevant experience will be considered. Knowledge, Skills & Abilities Strong coordination, interpersonal and communication skills. Ability to prioritize tasks with strong organizational skills. Knowledge of San Francisco County social services networks. Sensitivity to and possess a knowledge of services for seniors and adults with disabilities, homelessness/unstable housing, substance use disorders, and associated healthcare issues. Certified or familiar with HCV/Housing Quality Standards. Interacting with people from various social, cultural, economic, and educational backgrounds for the purpose of stabilizing or improving housing situations. Bilingual in Spanish, Tagalog, Mandarin/Chinese, or other language preferred. Functional knowledge of Microsoft Office Products. Demonstrated ability to utilize and monitor those internal and external client databases, as required by funders and Catholic Charities. Ability to design systems and processes to track data and monitor progress. Ability to perform duties with minimum supervision. Ability to communicate clearly in both verbal and written forms. Promote a positive professional and organizational image in the community. Sensitivity to seniors and people with disabilities and their needs. Knowledge of mandatory reporting requirements for people working with aging adults. Sensitivity in handling complex/confidential information. Ability to work under pressure. Ability to work as a member of a team. Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in cross‑cultural situations. Prerequisites Required Prior to the First Day of Employment TB Screening – Negative Tuberculosis Test. First Aid – Certificate. COVID‑19 Proof of Vaccination – Required. Is responsible for accurate and timely data entry that aids Performance and Quality Improvement (PQI) reporting and case record reviews. Is responsible for accurate and timely submission of case records. Physical Demands Frequent bending, standing, stooping, kneeling, reaching, twisting, and walking. Frequent lifting, pushing, and pulling. Frequent repetitive motions involving wrists, hands, and/or fingers. Medium work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects. Close visual acuity to perform an activity such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; and extensive reading. Driving is not required for this position. Working Conditions May need to enter private residences during client housing search or inspections. Occasionally exposed to outside weather conditions. Noisy and moderate work environment. May include contact with clients with mental health issues who demonstrate behaviors such as profanity, shouting, running away, self‑harm, and violence. Possible exposure to perfume or scents in personal care products used by employees, clients, and visitors. Exposure to outside environmental conditions with no effective protection from weather. Noise level in work environment can be moderate to excessive. Occasional need to interact with clients that may be expressing anger both appropriately and inappropriately, and to manage the client environment to avoid further escalation of any inappropriate behavior.
