Job Summary:

We are seeking a highly skilled and compassionate MDS RN Coordinator to join our team in Woodbury, New Jersey. As a key member of our nursing staff, you will play a critical role in ensuring the highest level of quality care and service to our patients.

Responsibilities:

• Coordinate and facilitate the Minimum Data Set (MDS) process, ensuring accurate and timely completion of assessments and reassessments

• Collaborate with healthcare team members to develop and implement comprehensive care plans that meet the unique needs of each patient

• Provide education and support to patients and families on disease management, treatment options, and self-care techniques

• Participate in Quality Improvement initiatives to ensure compliance with regulatory requirements and industry best practices

• Develop and maintain effective relationships with staff, patients, and families to promote a positive and respectful work environment

About the Role:

As a MDS RN Coordinator , you will have the opportunity to work closely with our multidisciplinary team to deliver exceptional care and service to our patients. Requirements:

• Current RN license in the state of New Jersey

• Minimum one year of experience in a post-acute care setting

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment with multiple priorities

• Strong organizational and time management skills

• Ability to comply with regulatory requirements and industry best practices
