We are seeking a compassionate and experienced Registered Nurse Assessment Coordinator to join our team at Tucker House Nursing and Rehabilitation. As a Registered Nurse Assessment Coordinator, you will play a vital role in ensuring that our residents receive high-quality, personalized care and attention.
Responsibilities:
Conduct thorough assessments of new residents to identify their needs, goals, and preferences
Develop individualized care plans that align with each resident's unique needs and goals
Collaborate with interdisciplinary teams to implement and monitor care plans
Provide ongoing assessment and management of resident conditions, including chronic illnesses and rehabilitation needs
Communicate effectively with residents, families, and healthcare professionals to ensure open and transparent communication
Maintain accurate and detailed records of assessments, care plans, and resident progress
Participate in quality improvement initiatives to ensure continuous quality improvement and excellence in care
