Pain Management (House Visits) - Nurse Practitioner (NP)

Overview Med-Care Providers delivers compassionate, high-quality care to patients in the comfort of their homes. We are expanding our specialty services and seeking a Nurse Practitioner with experience in Pain Management to conduct in-home visits and provide personalized, integrative pain care to seniors and medically complex patients. Position Overview We are looking for a skilled and empathetic NP who can assess, diagnose, and manage chronic and acute pain through in-home consultations. The ideal candidate will prioritize patient safety and evidence-based approaches, while coordinating care with primary providers and specialists. Responsibilities Conduct home visits for patients requiring pain management Perform comprehensive pain assessments and develop individualized care plans Prescribe appropriate therapies including medications, injections, and non-pharmacological approaches Monitor treatment effectiveness and adjust care plans accordingly Educate patients and caregivers on pain management strategies and safe medication use Maintain clear documentation in the EMR system Collaborate with physicians, specialists, and interdisciplinary teams Qualifications Licensed Nurse Practitioner in the state of Nevada DEA license with Schedule II-V prescribing authority Experience in pain management, geriatrics, or palliative care preferred Strong clinical judgment, compassion, and communication skills Reliable transportation and willingness to travel to patient homes Bilingual (English/Spanish) is a plus #J-18808-Ljbffr Med-Care Providers
