Countryside Manor | Fremont, OH
Full-Time | Day Shift
Countryside Manor is hiring Full-Time Licensed Practical Nurses (LPNs) for Day Shift in Fremont, Ohio!

Experienced LPNs and newly licensed nurses are encouraged to apply!
LPN ResponsibilitiesProvide direct nursing care to residents in a skilled nursing and long-term care setting
Administer medications and treatments as ordered
Monitor residents and assess changes in condition
Document resident care accurately and timely
Communicate changes in resident condition to physicians and appropriate members of the care team
Follow individualized resident care plans
Provide guidance and support to CNAs and other members of the nursing team
Respond promptly to resident needs and concerns
Maintain resident dignity, privacy, comfort, and safety
Follow infection control, safety, and facility procedures
Communicate effectively with residents, families, physicians, and healthcare team members
Work collaboratively with the nursing team to provide excellent resident care
