Pay Rate: $2,140 – $2,240/week

Position Overview

We are seeking a Licensed Master Social Worker (LMSW) to provide social work services in an inpatient hospital setting, including psychosocial assessments, discharge planning, and support for patients and families.

Required Qualifications

• Master’s Degree in Social Work (MSW).

• Active New York State LMSW license.

• Inpatient hospital social work experience.

• Discharge planning experience.

• Experience with goals-of-care and end-of-life planning conversations.

• Strong communication skills and experience conducting psychosocial assessments.

• Oncology experience preferred.

Responsibilities

• Provide social work services to patients and families in an inpatient hospital setting.

• Conduct psychosocial assessments and identify patient and family needs.

• Assist with discharge planning and coordinate appropriate resources.

• Participate in goals-of-care discussions and end-of-life planning.

• Support patients and families through complex medical situations.

• Collaborate with physicians, nurses, case managers, and other members of the care team.

• Provide referrals and connect patients with appropriate community resources.

• Document assessments, interventions, and care planning activities.

• Support oncology patients and their families as appropriate.
