Description

• Conduct patient evaluations in either an inpatient or outpatient environment and develop customized treatment plans
• Implement physical therapy interventions targeting mobility, strength, balance, and overall function
• Collaborate with a multidisciplinary team, including nurses, physicians, case managers, and other rehabilitation specialists, to achieve optimal patient outcomes
• Guide patients and their families through therapeutic objectives, discharge processes, and appropriate home exercises
• Accurately document evaluations, treatments, and patient progress per our facility and relevant regulations
• Engage in interdisciplinary conferences and care team rounds to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from a recognized program

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory

Skills

• Strong preference for prior experience
• Ability to treat complex medical and mobility challenges
