Loudoun Home Care is looking for a compassionate, dependable, and clinically confident RN/LPN to join our growing team. This is an opportunity to provide meaningful one-on-one nursing care while building a trusting relationship with a client and their family.

PAID TRAINING & ORIENTATION PROVIDED

You will receive training and hands-on orientation before independently starting your shift.

We want you to feel confident and prepared before you are responsible for the client independently. Our team will provide the appropriate training and orientation based on the client’s care needs, including specialized care requirements.

What You'll Do

As an RN/LPN, you will provide and coordinate skilled nursing care based on each client's individual needs and plan of care.

Your responsibilities may include:

• Perform comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing care, including medications, treatments, wound care, and other ordered services.
• Provide specialized care for clients with tracheostomies and G-tubes .
• Monitor clients closely and recognize changes in condition that may require intervention.
• Communicate significant changes and clinical concerns promptly to physicians and the appropriate care team.
• Maintain accurate, timely, and complete clinical documentation.
• Coordinate care with physicians, families, caregivers, therapists, and other healthcare professionals.
• Complete required supervisory and reassessment visits.
• Educate clients and families regarding medications, treatments, safety, infection prevention, and ongoing care needs.
• Promote a safe, respectful, and comfortable home environment.
• Follow applicable Virginia regulations, professional nursing standards, infection-control practices, and agency policies.
• Participate in staff education, training, and quality-improvement initiatives.

What We're Looking For

• Active Virginia or Compact RN/LPN license
• Minimum 1 year of nursing experience
• Current CPR/BLS certification
• Experience caring for clients with tracheostomy and G-tube
• Home care, private duty nursing, community health, or similar experience preferred
• Strong clinical judgment and assessment skills
• Excellent communication and documentation skills
• Ability to work independently while remaining connected with the care team
• Dependable, compassionate, patient-focused, and professional approach
• Comfortable communicating with clients, families, physicians, and other healthcare professionals
