Admissions Registered Nurse
Chapters Health System
Chapters Health System is seeking an RN Admissions nurse to assess patients in their homes or hospitals and initiate care with Care Navigation. The role includes strong emphasis on transition of care and representing the organization in clinical settings. Requirements include an active RN license, 1+ year nursing experience (hospice or hospital preferred), EMR/EHR experience, and valid driver’s license.
