Family Practice - Geriatrics Physician

Primary Care Physician Locations: Dartmouth, Leominster or Springfield, MA Compensation: $275,000 $300,000 annually, based on experience Join a mission-driven healthcare organization delivering comprehensive, community-based care to older adults through a PACE (Program of All-Inclusive Care for the Elderly) model. Why This Role Exceptional Work-Life Balance: Monday Friday schedule, 8-hour days, flexible hours, and an optional administrative day with no direct patient care Low Patient Volume: Average of 4 patients per day with a panel of 65 Team-Based Care: Work closely with an interdisciplinary team including nursing, therapy, social work, and care coordination Mission-Focused Practice: Help frail elders remain safely in their homes and avoid nursing home placement Professional Support: Strong peer mentorship and leadership engagement Key Responsibilities Provide primary, preventive, urgent, and chronic care to PACE participants Conduct initial and biannual assessments and contribute to individualized care plans Participate actively in interdisciplinary team (IDT) meetings Coordinate specialty care and transitional care services Deliver care across settings including clinic, home, and inpatient facilities Participate in limited after-hours call rotation Maintain timely and accurate clinical documentation Qualifications MD or DO licensed in Massachusetts; DEA required Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatrics (geriatrics preferred) This opportunity offers a sustainable, rewarding practice environment where physicians can focus on patient-centered care and long-term impact.
