Care Manager RN

UCHealth is seeking a Care Manager RN to coordinate complex patient care across our Colorado-based health system. In this role, you’ll use your clinical expertise to assess needs, develop and manage care plans, and ensure safe, timely transitions from hospital to home or post-acute settings. You’ll collaborate closely with physicians, nurses, social workers, and community partners, while educating patients and families to promote self-management.
