SUMMARY OF JOB

LPN Personal Care Nurse assesses the physical needs of Personal Care resident and takes responsibility for the total care of the residents, assessing changes in residents and making appropriate responses based on assessment. Works in collaboration with other healthcare professionals, residents and their families. Acts as a role model. Recognizes the value of each person as an individual. Supports and Promotes Resident strengths and abilities, capacity for social contribution, unique values, preferences, living habits and autonomy of choice. Embraces a vision of aging that is meaningful and relationship based

RECRUITMENT REQUIREMENTS

Licensed Graduate Practical Nurse; Knowledge of Geriatrics; Good physical assessment skills; One-year experience in Nursing or LTC

Ability to work on a team addressing the holistic needs of the Geriatric population
