Up to $5000 Sign On Bonus!

Evening Shift, 3 times a week

Also Available:

Night Shift- Mon- Thursday

Day Shift- Alternate weekends

Deer Meadows Personal Care Home is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals in Philadelphia, Pennsylvania. As an LPN, you will play a vital role in providing exceptional care to our residents in our Personal Care Home.

About the LPN for Personal Care Role

As a Licensed Practical Nurse (LPN), you will be responsible for:

• Providing high-quality nursing care to residents in our Personal Care Home,

• Developing and implementing individualized care plans in collaboration with the interdisciplinary care team

• Monitoring and reporting changes in resident condition, including any concerns or issues that may arise

• Educating residents, families, and caregivers on care instructions

• Maintaining accurate and complete records of resident care, including documentation of resident interactions

LPN for Personal Care Requirements

• Must be a Licensed Practical Nurse (LPN) with a current license to practice in the state of Pennsylvania

• Prior experience in a healthcare setting

• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently

• Excellent communication and interpersonal skills, with the ability to work collaboratively with the interdisciplinary care team

• Compassionate and respectful attitude towards residents, families, and colleagues
