Registered Nurse (RN) - Case ManagerWe are seeking a dedicated and experienced Registered Nurse (RN) for a Case Manager position. The ideal candidate will have recent acute care discharge planning experience and will be responsible for coordinating patient care and ensuring smooth transitions from hospital to home or other care facilities.General InformationUnit Specifics: Number of Beds: 590Patient Ratios: 1 RN to 25 patients, variesUnit Staffing Description: RN, SW, and CMADocumentation System: Meditech, Midas, WellskyScheduling Requirements: Monday to Friday from 8:00 AM to 4:30 PM with rotating weekendsFloating Policy: May float to any unit as neededQualificationsMust possess recent acute care discharge planning experience.Strong communication and organizational skills.Ability to work collaboratively in a team environment.Ability to adapt to changing patient care needs and priorities.Dress CodeBusiness casual attire is required.If you meet the qualifications and are interested in joining our team as a Case Manager, please submit your application for consideration.Client Details Address 2316 E MEYER BLVD State MO Zip Code 64132
