Job Details

We are seeking a highly skilled and dedicated Family Medicine Physician to join our progressive healthcare team. This permanent position offers the opportunity to work in a dynamic, patient-focused environment where you will be able to make a significant impact on the health and well-being of individuals, families, and communities. The ideal candidate will have a strong background in chronic disease management, EMR proficiency, behavioral health collaboration, and preventive care delivery.

Responsibilities:

As a Family Medicine Physician, you will be expected to:

• Provide comprehensive and continuous primary care to a diverse group of patients, including individuals, families, and communities.
• Diagnose, treat, and manage a wide range of health conditions, with a particular focus on chronic disease management.
• Utilize electronic medical records (EMR) system to maintain accurate and up-to-date patient records, facilitate communication with other healthcare providers, and support data-driven decision-making.
• Collaborate closely with behavioral health professionals to ensure that patients receive integrated, whole-person care.
• Develop and implement preventive care strategies to promote health and prevent disease.
• Educate patients and their families on disease prevention and health promotion strategies.
• Participate in ongoing professional development to stay abreast of the latest advancements in family medicine.

Qualifications:
To be considered for this position, you must have:

• A Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited medical school.
• Completed a residency in Family Medicine.
• Board certification in Family Medicine.
• A valid and unrestricted license to practice medicine.
• A minimum of five years of clinical experience in Family Medicine, with a strong emphasis on chronic disease management, EMR proficiency, behavioral health collaboration, and preventive care delivery.
• Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and other healthcare professionals.
• A commitment to providing high-quality, patient-centered care.
