Description

• Conduct thorough evaluations for patients in inpatient or outpatient settings, developing tailored treatment plans according to individual needs
• Implement physical therapy interventions targeting mobility, strength, balance, and functionality
• Work closely with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal recovery outcomes
• Advise patients and their families on therapeutic goals, discharge preparations, and home exercise regimens
• Maintain accurate documentation of evaluations, treatments, and patient progress, in line with facility standards and regulations
• Engage in care team rounds and interdisciplinary meetings, promoting coordinated and comprehensive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized credentials (e.g., in geriatrics or neurology) are a plus but not mandatory

Skills

• Prior experience is strongly preferred
