Responsible for managing the activities of the multi-disciplinary team providing care to the patient, in addition to monitoring the provision of therapeutic services through written and verbal communication with all disciplines. Ensure that all visits scheduled and provided are authorized and covered by physician orders. Responsibilities include review of patient visits weekly and providing plan to Supervisor Patient Care and team assistant, including anticipated weekend visits. Inform team assistant of any schedule changes and communicates caseload and patient care issues to Supervisor Patient Care. Coordinates care with care team assigned to caseload, participates in care coordination communication and documentation thereof. Self manages visits weekly by triaging by risks associated with potential rehospitalizations. Participates in quarterly performance improvement action plans. Complete patient documentation timely as indicated by supervisor with accuracy.Minimum Qualifications:1 year - Registered Nurse; Or - 1 year - Licensed Practical Nurse;1 year - Home Care.Florida driver's license is required within 30 days of hire.Education:Associates - Nursing; Or - Diploma - Nursing;Bachelors - NursingCredentials:Basic Life SupportRegistered NurseDrivers LicenseBLS (Basic Life Support) - Certification;RN (Registered Nurse) - License;Driver's License - State of FloridaLocation: HomeCare LargoStatus: Full Time, Exempt: YesShift Hours: 8:00 AM - 5:00 PM Shift: Shift 1Weekend Work: Every OtherOn Call: YesHow often will this team member be working remotely?
