Physical Therapist – Home Healthcare - Hospice in Sunnyvale, California, United States. Start date: 06/15/2026. Travel assignment lasting 26 weeks. Weekly pay range: $2,541–$2,708 per week. Responsibilities
- Evaluate and treat patients using medically prescribed physical therapy programs in a home setting.
- Develop and implement plans of care aimed at restoring function and minimizing disability, within the patient’s capabilities.
- Document observations, clinical findings, problems, skilled interventions, goals, and discharge plans in a timely manner.
- Supervise Physical Therapy Assistants as appropriate and participate in initial admission/evaluation visits.
- Notify the assigned Clinical Team Manager of OASIS scores within established timelines and collaborate with the care team to adjust care as needed.

Requirements
- One year of Home Health experience
- Ability to perform initial evaluations and ongoing PT services within a home health/hospice setting
- Strong documentation and communication skills to support timely care planning and discharge
- Willingness to work within the listed territory and meet the schedule flexibility described (including one weekend every few weeks)
