In this role, you’ll provide individualized care to clients in their homes while maintaining a schedule that works for you. As our territory continues to grow, additional hours may become available. Key Responsibilities: Provide nursing services to patients in a home setting. Perform routine nursing care to patients as prescribed by the healthcare provider. Monitor patient health and maintain accurate records of vital signs and health progress. Educate patients and their families on health maintenance and disease prevention Requirements : Valid LPN license in the state of Minnesota. Experience in home health care or similar settings is preferred. Strong interpersonal skills and a passion for providing empathetic, respectful care.
