Responsibilities:

• Conduct comprehensive home health assessments and develop individualized care plans
• Perform OASIS assessments and ensure documentation accuracy in compliance with regulations
• Administer medications, wound care, IV therapy, and patient education
• Collaborate with interdisciplinary teams to coordinate patient care in community settings
• Utilize EMR systems such as Homecare Homebase for documentation and reporting
