Patient Care Coordinator

Department : GCH Patient Management Location: Golisano Children's Hospital Location of Job : US:NY:Buffalo Work Type : Per-Diem Scheduled Work Hours: Variable Shift 9 Job Description Has the responsibility of collaborating with members of the interdisciplinary team to coordinate care, facilitate utilization and resource management and discharge planning functions, working to achieve optimal outcomes for the patient in the acute care setting. Case management is a process that efficiently and effectively aligns patient needs/issues with resources to meet quality, clinical and cost outcomes. Education And Credentials Current New York State registration as a Registered Professional Nurse required. Bachelor of Science degree in Nursing required within five (5) years of hire to the position. Certified Case Manager (CCM) preferred. Employees in the job title prior to 3/29/10 are grandfathered and not required to obtain their BSN. Basic Life Support (BLS) required upon hire. Current NYS registration as a Registered Professional Nurse/RN Permit and American Heart Association-Basic Life Support (BLS) required upon hire and must be maintained thereafter.PRI certification required and will be completed during orientation period and maintained thereafter for new staff. Current staff will have until 03/31/2026 to obtain certification. Experience 2 years of acute care Nursing Experience required. 2 years of experience in any combination of case management, home care and utilization review preferred. Knowledge of and experience with Interqual Criteria set and software systems preferred. Working knowledge of Microsoft Office Suite and Outlook required. Use of computer applications related to health care industry required. **For Registered Nurse clinical positions, internal applicants must have been employed in the same job title as the per diem position for which they are applying or in a comparable area of practice. Internal applicants must have a minimum of nine (9) continuous months of comparable work experience within two (2) years of the time the per diem job is posted. External applicants must have a minimum of one (1) continuous year of comparable work experience within two (2) years of the time the per diem job is posted.
