Job Opportunity: RN Case Manager

Our organization is on the lookout for a dedicated RN Case Manager to oversee holistic patient care across various healthcare environments. In this essential position, you will:

Conduct comprehensive clinical evaluations.
Design and implement personalized care plans.
Facilitate safe transitions between different care settings.
Work collaboratively with a multidisciplinary team.
Provide education to patients and their families.
Monitor the utilization of services.
Maintain accurate documentation in the electronic medical record (EMR) system.
