Misericordia Heart of Mercy is seeking experienced Case Managers / QIDPs in Chicago, IL. You will work closely with residents, families, and an interdisciplinary team to build individualized programs and support residents toward greater independence. Responsibilities include overseeing programs, coordinating services, and guiding residents to achieve personal goals while advocating for rights, dignity, and quality care.
