$5,000 Sign on Bonus Speech Language Pathologist (SLP) – Home Health Services (Full Time, Part Time, or Per Diem) Locations: Duval/Clay and St. John's Counties. Job Purpose Provide evaluative, consultative, and treatment interventions to home‑health patients referred by a licensed physician in their place of residence, maintaining effective communication with Home Health partners, United Rehab partners, patients, families, and other internal and external customers. Key Responsibilities Complete evaluations, assessments, and treatments in accordance with policy and individualized care plans within established timelines. Collaborate with the interdisciplinary team and attending physician to develop, integrate, and manage patient care plans, making referrals as needed. Demonstrate knowledge of home care and competency in discipline‑specific patient care skills. Recognize and respond to patient care priorities with initiative and sound judgment. Help train or orient staff members. Utilize company teaching materials for patient/family education. Report alleged or suspected violations to supervisors until resolution. Comply with the corporate compliance program. Meet agency productivity standards and use time and resources efficiently. Participate in center/agency surveys, mandatory in‑services, case conferences, staff meetings, and improvement programs. Follow safety regulations and report occupational hazards. Maintain current license, certification, and clinical records to meet federal, state, and agency regulations. Be self‑directed, prioritize tasks, manage time, and meet strategic goals by targeted deadlines. Licensure, Certification, and Education Requirements Valid professional license in the state of practice. Graduate of an ASHA‑accredited school, or foreign‑trained with a program deemed substantially equivalent by an ASHA‑approved credentials evaluation organization. Preferred Skills and Experience Prior home‑health experience is preferable.
