Position Summary

WC Health, a Behavioral Health Company, is seeking a dedicated and compassionate Junior Case Manager to join our team. The Secondary Case Manager works collaboratively with Primary Case Managers and the interdisciplinary team to support members in the behavioral health housing program.

This position assists with member monitoring, treatment plan coordination, needs assessments, care coordination, and member interventions. The Secondary Case Manager must demonstrate a person-centered approach, strong communication skills, and the ability to work effectively with Medicaid members who may have complex behavioral health and social service needs.

The ideal candidate is a self-starter who works well under minimal supervision, demonstrates sound judgment, and is committed to providing respectful, trauma-informed, and person-centered services.

Essential Duties and Responsibilities

The Junior Case Manager's daily responsibilities include, but are not limited to:

• Assist Primary Case Managers with monitoring and management of members' treatment plans and overall care needs.

• Maintain oversight of members in the housing program, including verifying treatment compliance, medication management, and appropriate follow-up on identified concerns.

• Provide support and care coordination for Medicaid members, including assisting with access to healthcare, behavioral health services, and community resources.

• Conduct member Needs Assessments through virtual and/or in-person meetings to identify strengths, needs, barriers, goals, and appropriate support services.

• Coordinate with pharmacies regarding member medications, prescription needs, and medication-related concerns, within the scope of the position.

• Coordinate telehealth, clinical, and other healthcare appointments as needed.

• Document member interactions, interventions, assessments, referrals, and follow-up activities accurately and timely within the Electronic Health Record (EHR).

• Monitor members' overall well-being, including personal hygiene, room cleanliness, sobriety, and adherence to applicable program expectations, while maintaining dignity and respect.

• Utilize active listening skills, person-centered communication, and de-escalation techniques when interacting with members experiencing distress, conflict, or challenging situations.

• Assist in identifying and addressing member barriers, including transportation, healthcare access, housing stability, and other social determinants of health.

• Communicate and collaborate effectively with Primary Case Managers, the Program Manager, Housing Staff, clinical personnel, and other members of the interdisciplinary team.

• Maintain confidentiality and comply with applicable HIPAA privacy regulations, organizational policies, and all relevant laws and regulations.

• Perform duties at multiple assigned locations, including traveling between two housing program locations as needed. Must have your own transportation.

• Complete other duties and responsibilities as assigned by leadership.

Required Qualifications and Skills

• Education: High School Diploma or equivalent required.

• QMHA Certification/Qualification Required: Candidates must meet QMHA requirements and possess a bachelor's degree in Human Services, Psychology, Social Work, or a related field. Candidates must be able to provide documentation of their educational qualifications and meet all applicable organizational and regulatory requirements.

• Experience working with Medicaid members or individuals receiving Medicaid-funded behavioral health services is preferred.

• Knowledge of HIPAA privacy regulations and the importance of protecting confidential member information.

• Understanding and ability to apply person-centered care principles when working with members.

• Knowledge of and ability to utilize de-escalation techniques to support members during challenging or emotionally heightened situations.

• Strong active listening, verbal, and written communication skills.

• Ability to conduct and document member Needs Assessments through virtual and/or in-person interactions.

• Ability to work effectively with individuals experiencing behavioral health challenges, varying levels of functioning, and complex social needs.

• Ability to work independently with minimal supervision while demonstrating initiative, accountability, and sound judgment.

• Self-starter with the ability to prioritize tasks, manage responsibilities, and follow through with member needs and assigned duties.

• Proficiency in Google Suite, including Google Docs, Google Sheets, Google Drive, and other technology platforms used in the workplace.

• Ability to learn and effectively use Electronic Health Records (EHRs), telehealth platforms, and other required technology.

• Strong organizational skills and attention to detail, particularly in documentation and care coordination.

• Ability to maintain professional boundaries, confidentiality, and respectful communication with members and coworkers.

• Must have reliable transportation and the ability to travel between two assigned housing program locations as needed for job responsibilities.

• Ability to work collaboratively as part of a multidisciplinary team.

Physical and Technical Requirements

• Must have sufficient manual dexterity to perform essential job functions, including writing, reading, typing, and operating a computer and other office equipment.

• Ability to read, understand, and complete written documentation, member records, assessments, and other job-related materials.

• Ability to type and enter accurate documentation into electronic systems in a timely manner.

• Ability to use computers, mobile devices, and other technology required to perform assigned duties.

• Ability to perform the essential functions of the position, with or without reasonable accommodation.

Pay: $17/hr
