Licensed Clinical Social Worker

Job Summary As part of the Care Management team, the Social Worker combines social expertise with knowledge of community resources, and applies principles of quality management, discharge planning and the management of clinical/financial resources as a facilitator and consultant to the multidisciplinary patient care team. The Care Manager predicts, facilitates, and evaluates the transition of patients across the care continuum and ensures linkages with post‑acute discharge providers. Interpret, analyze, and apply relevant data to prioritize and determine a course of action appropriate to meet patients’ management needs. Assess, plan, implement and evaluate strategies that ensure appropriate utilization of clinical resources and management of length of stay. Assess, plan, implement and evaluate effectiveness of the discharge plan for the caseload of patients. Meet all organizational requirements and demonstrate initiative to establish and achieve personal and professional goals. Provide effective customer service behaviors as defined by the organization’s mission, vision and values. Create and implement discharge plans for every admitted patient, assessing medical, functional, psychosocial, legal/financial and safety/status factors. Develop discharge plans tailored to patients’ needs and problems; collaborate with physicians, nurses and multidisciplinary team for recommendations. Collaborate and communicate with Social Workers as an ongoing process. Make referrals to Social Worker as appropriate. Identify and refer patients and families to specific resources such as agency and private caregivers, equipment, mental health, transportation, medical and housing resources, and educational materials. Implement discharge plans and referrals; identify and resolve delays and obstacles; act as primary leader of the discharge plan. Monitor length of stay and ancillary resource use; identify opportunities for process improvement and recommend actions. Communicate following the chain of command regarding proper utilization of resources, physician concerns, and length of stay activities. Provide information regarding denials/approvals; expedite the peer‑to‑peer process through collaboration with physicians and insurance companies for post‑acute activities. Communicate denials verbally and in writing to patients, families and physicians as needed for post‑acute services. Enter all pertinent data (discharge plan) into data collection system per policy and established process. Participate in clinical performance improvement activities, complete readmission interviews with patients and families, and enter information into appropriate systems. Interpret and negotiate with state, local and federal agencies to optimize placement in the most appropriate setting, aligning patient needs with desired level of care. Work within CMSA standards of practice. Achieve proficiency in core job responsibilities within 90 days. Develop, create and oversee a social work internship program for baccalaureate students. Participate in organization committees and report back to department activities. Support the department by managing projects such as guardianship cases. Explore and communicate best practices within the department. Perform all other duties as assigned. Education Minimum Required Education: Master’s of Social Work (MSW) Experience Minimum of 5 years of clinical social work experience required. Experience with discharge planning, home health, and ambulatory care preferred. Experience may be waived if completed Chesapeake Regional Medical Center Internship Program. Licensure & Certifications Licensed Clinical Social Worker CCM or ACM required within 2 years of eligibility for the exam.
