Position: Interim Community Health Associate Salary Range: $55,000 - $68,000 (annualized) Reports to: Chief Program Officer For 67 years, LSA Family Health Service, a community-based nonprofit in East Harlem, has supported thousands of diverse and immigrant families striving to build better lives. Position Overview: We are seeking an Interim Community Health Associate to maintain our management capacity during our Community Health Director’s parental leave. This temporary role is crucial for maintaining the continuity of our civic engagement initiatives, helping to coordinate pop-up health events and systematically ensuring the fulfillment of health-related programs and deliverables. We are open to considering both full-time and part-time arrangements. Note on Position Duration: While this is an interim position intended to provide parental leave coverage, exceptional candidates may be considered for a permanent position within the department depending on organizational growth, funding availability, and the team’s workload. A Typical Day Your morning begins by evaluating case capacities and timelines within Family Support Services to ensure community members who depend on you don't fall through the cracks. You quickly pivot to finalizing health clinic calendars and educational flyers with clinical partners like Mt. Sinai or Memorial Sloan Kettering, ensuring that clients have seamless access to upcoming care. By midday, a crisis hits: Elena is facing an emergency eviction and severe maintenance delays with her apartment’s property management. You instantly step in to triage the emergency and keep Elena and her family safely housed. In the afternoon, you turn that frontline victory into data-driven impact by auditing environmental health tracking sheets to align with new data capture mandates and compiling Social Determinants of Health screenings for the monthly United Way report. You finish the day at an East Harlem neighborhood coalition meeting, mapping out fresh outreach strategies to welcome the next family to the center. The Location This is an onsite location at our Center in East Harlem with some travel to other sites in the neighborhood and occasional travel to other sites in New York City for events, training and collaboration with partner organizations. Skills & Experience Bachelor’s degree in public health, social work, public administration, or a related field. 3–5 years of progressive program management experience within a non-profit or community health setting required. Demonstrated experience managing government or foundational grant metrics (experience with United Way or municipal tracking frameworks is strongly preferred). Familiarity with the East Harlem community landscape and public housing/voucher systems (NYCHA, Section 8) is preferred. Technical proficiency with data systems or direct experience utilizing a database for program auditing is a plus.
