Pharmacy Supervisor (Oncology)

Summary The incumbent is responsible for development, implementation, management, and reporting of outcomes for the Oncology Drug Stewardship Program. They will institute improved auditing to ensure the safe and effective use of drugs, manage requests for oncology medications, and collaborate with the Formulary Program Manager, Pharmacoeconomics Program Manager, and Pharmacy Procurement Technicians to develop and enforce VA formulary guidelines and contain drug costs. Qualifications Citizenship – United States citizenship is required. Non‑citizens may be appointed only when recruitment of qualified citizens is not possible in accordance with VA policy. English Language Proficiency – Pharmacists must demonstrate spoken and written proficiency as required by 38 U.S.C. 7402(d) and 7407(d). Education Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a bachelor’s degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs must provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, including passing the FPGEE and TOEFL iBT. Licensure Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States, or the District of Columbia. The pharmacist must maintain current registration if required for licensure. Licenses that have ever been revoked, suspended, denied, restricted, limited, or subject to probation may be considered only in accordance with VA Handbook 5005, Part II, Chapter 3, Section B, Paragraph 16. Grade Determinations – GS‑13 Pharmacy Supervisor (Oncology) experience. Must have at least one year of experience equivalent to the next lower grade level. Knowledge, Skills, and Abilities (KSAs) Ability to effectively supervise subordinate staff. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards pertaining to pharmacy. Skill in managing people or programs. Preferred Experience Completion of a Post‑Graduate Year 1 (PGY1) residency and a Post‑Graduate Year 2 (PGY2) residency, preferably with a specialization in Oncology. Minimum of three years of professional experience in Oncology Pharmacy. Board Certification in Oncology Pharmacy (BCOP) is strongly preferred. Reference – For more information on qualification standards, visit Physical Requirements Heavy lifting (45 lbs and over), moderate lifting (15‑44 lbs), light lifting (15 lbs and under). Reaching above shoulder, use of fingers, both hands, standing, repeated bending. Vision: near 13‑16 inches to Jaeger 1‑4, far 20/20 (one eye) to 20/40 (other eye). Depth perception, basic color discrimination, hearing (audiology aid may be permitted). Environmental factors include constant noise, dust, keyboard/mouse use, working closely with others, working alone, and protracted or irregular hours. Physical Capability – The ability to maintain the above lifting, vision, and environmental tolerance is required. Work Schedule – 7 AM to 3:30 PM, Monday to Friday. Telework: Not available. Financial Disclosure: Required. Duties Develop, implement, and maintain the Oncology Drug Stewardship Program to improve patient outcomes on oncology drugs. Collaborate with the Formulary Program Manager to interpret and implement drug formulary restrictions, practice guidelines, manage drug shortages, and evaluate non‑formulary or off‑label drug requests. Collaborate with the Pharmacoeconomics Program Manager to identify and implement cost savings and avoidance activities for established or emerging oncology therapies. Collaborate with Hematology‑Oncology, MTU, Nursing, and Clinical Nutrition Services to proactively create, review, and update local oncology disease treatment guidelines, clinical pathways, policies, and dosing protocols. Monitor utilization rates for oncology drugs and initiate procedures to optimize use. Maintain clinical oversight of patients receiving oncology drugs to ensure safe and effective utilization. Document all recommendations and interactions in the electronic health record (EHR) as progress notes in an appropriate format to maximize clinical pharmacy workload capture. Identify, address, and resolve drug‑related problems in a consistent and coordinated process. Complete medication reconciliation and provide patients with an updated medication list in compliance with current policies. Participate in the Adverse Drug Event Reporting Program and report medication errors to promote Veteran patient safety. Provide in‑depth review of Prior Authorization Drug Requests (PADRs) using the PADR consult process to document approval or disapproval for non‑formulary or restricted drugs. Initiate orders for medications, medication changes, and lab tests as permitted by scope of practice and medical center policy or protocol. Maintain advanced training and knowledge of pharmaceutics, pharmacodynamics, and pharmacotherapeutics. Monitor and assess the outcomes of oncology therapy including interpretation of laboratory data and appropriate diagnostic procedures. Facilitate and coordinate clinical activities of pharmacists within their scope of practice to promote consistently high pharmaceutical care for all patients under their care. Assess and provide comprehensive medication and disease‑state counseling to veteran patients and families to ensure safe medication use and disease‑state outcome goals are met. Provide drug information to health professionals and pharmacy staff regarding changes in practice, formulary management, and new modalities related to oncology drugs. Resolve questions posed by physicians, consultants, and other healthcare providers regarding drug therapy appropriateness, stability, compatibility, contraindications, adverse effects, drug‑drug/drug‑food/drug‑disease interactions, drug‑use criteria, and formulary alternatives. Precept pharmacy students and residents. Promote pharmacy clinical service philosophy of multidisciplinary teamwork and customer service, ensuring patient‑focused decisions. Maintain continuous readiness for internal and external quality surveys. Ensure hazardous medications are handled in accordance with departmental policies, procedures, and state and federal laws and regulations. Ensure VA Puget Sound Pharmacy services meet or exceed all applicable facility and VHA policies, FDA and DEA regulations, federal and state laws, and the Joint Commission standards for oncology drugs. Supervise assigned Clinical Pharmacist Practitioners (CPPs), including assigning schedules, managing leave, and conducting appraisals of staff performance. Recruit and select new staff using performance‑based interviewing techniques and equal employment opportunity principles. Incorporate continuous quality improvement (CQI) philosophy into all areas of the pharmacy. Assist medical staff in developing, conducting, and presenting medication use evaluations, outcome measures, and basic research. Participate as a preceptor for pharmacy PGY‑1 and PGY‑2 residents or projects. Assist with investigational new drug processes and facilitate pharmacist involvement in pharmacotherapy research and scholarship.
