About This Role The Hospital Safe Discharge RN Case Manager initiates proactive outreach to facility staff to manage discharges, working collaboratively to resolve discharge barriers and ensure a smooth transition for patients. Highlights Pay: $29 - $52/hour Location: Baltimore, MD Schedule: Full Time, Day shift Residency: Must reside in the U.S. What You'll Do Assess barriers to a safe and efficient discharge process. Coordinate and drive an efficient discharge process with stakeholders. Demonstrate knowledge of acute inpatient case management and care coordination. Manage assigned case load efficiently utilizing time management skills. Prepare and lead periodic rounds to discuss case needs and prioritize discharges. Utilize electronic platforms and EMR systems for patient care coordination. Confer with Medical Director or leadership for complex case reviews. Identify and communicate a member's level of risk and participate in planning. Collaborate with hospital staff, patients, families, and community services. Support hospital staff in facilitating member transitions to post-acute care.
