RN Case Manager: Hospital Care & Discharge

UF Health

UF Health is seeking an RN Case Manager to assess, plan, implement and monitor care for a designated patient group within our hospital teams. You will collaborate with physicians, social workers, PT/OT, pharmacists, and dietary staff to coordinate resources and ensure a smooth transition from acute care.

The role emphasizes patient progression, safe discharge planning, and efficient care utilization. Florida RN licensure is required; a BSN is preferred and relevant case-management experience is
