Registered Nurse Case Manager

Providing telephonic complex case management, the full-time Arizona Licensed RN Case Manager will coordinate care for adults with complex healthcare needs, partnering with members, caregivers, and providers to facilitate access to necessary services and resources in a remote setting. Key responsibilities Manage individual patients across the healthcare continuum to achieve optimal clinical and operational outcomes Collaborate with interdisciplinary teams to develop and communicate effective patient discharge plans Evaluate medical necessity and appropriateness of care while resolving barriers to effective patient care Required qualifications Current Registered Nurse (R.N.) license in Arizona Bachelor's degree in case management or healthcare 3-5 years of clinical experience in care management or acute care settings Knowledge of community resources and resource/utilization management Basic Life Support (BLS) certification for acute care settings
