Physical Therapist (Full-Time)

Birchwood Terrace Rehabilitation and Healthcare has an exciting opportunity for a Full-Time Physical Therapist (PT) in Burlington, Vermont !

We are looking for a dedicated PT to join our in-house rehabilitation team of outstanding professionals who provide person-centered care to our residents and patients. Physical Therapist Responsibilities

• Consult with patients and residents to understand their individual needs, concerns, and rehabilitation goals.

• Perform comprehensive physical therapy evaluations and interpret evaluation findings and test results.

• Develop individualized treatment plans using evidence-based physical therapy techniques and interventions.

• Create personalized rehabilitation programs designed to improve function, mobility, strength, balance, and independence.

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, increase mobility, and promote healing.

• Provide treatment in accordance with each patient's established plan of care.

• Monitor patient responses to treatment and modify interventions as clinically appropriate.

• Collaborate with the interdisciplinary team to promote coordinated, high-quality patient care.

• Provide education and guidance to patients, residents, families, and caregivers regarding treatment plans, exercises, and functional goals.

• Maintain accurate, thorough, and timely clinical documentation in accordance with company policies and applicable state and federal regulations.

• Communicate effectively with nursing, therapy, medical, and other clinical team members.

• Attend required meetings and participate in additional duties as assigned by the Director of Rehabilitation.

Physical Therapist Skills & Qualifications

• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.

• Current knowledge of physical therapy treatment practices, techniques, and interventions.

• Familiarity with Net Health electronic documentation systems preferred.

• Strong clinical, communication, and interpersonal skills.

• Ability to work effectively with patients and residents with a variety of needs, abilities, and backgrounds.

• Strong organizational skills and attention to detail.

• Ability to collaborate effectively with patients, families, caregivers, and members of the interdisciplinary team.

• Commitment to compassionate, person-centered care.

Physical Therapist Requirements

• Active and valid Physical Therapist license in the State of Vermont .

• Must be in good standing with all applicable regulatory agencies and licensing boards.
