As a Patient Care Coordinator , you will serve as a primary point of contact for patients throughout their infusion therapy journey, ensuring they receive exceptional service, clear communication, and a seamless experience from start to finish.

What You’ll Do:
- Serve as a dedicated point of contact for patients throughout the referral, scheduling, and treatment process.
- Keep patients informed of their treatment status, appointment details, and next steps.
- Assist patients with navigating the infusion process and answer non-clinical questions.
- Coordinate with internal clinical, pharmacy, and administrative teams to ensure a smooth patient experience.
- Provide exceptional customer service and support while maintaining patient confidentiality and compliance standards.
- Help identify and resolve patient concerns in a timely and professional manner.
- Maintain accurate and timely documentation related to patient communication and coordination.
- Travel between locations as needed.
