RN Case Manager

Role: The RN Case Manager provides end-of-life hospice care in patients' homes or community settings, supervising hospice aide staff and coordinating patient services. Key Responsibilities Assess, plan, implement, and evaluate patient care needs during on-call hours. Report activities and maintain detailed clinical documentation. Assist with medication and supply delivery outside regular hours. Participate in interdisciplinary team meetings and collaborate with physicians. Admit eligible patients for hospice services in coordination with the triage team. Develop personalized care plans addressing patient problems and goals. Support the use of the 4Ms (What Matters, Medications, Mentation, Mobility) and provide Age-Friendly Care. Qualifications & Skills Current RN licensure in the practicing state. Valid driver’s license and auto insurance. At least two years of RN experience, with hospice or palliative care experience preferred. Strong clinical judgment, excellent communication, compassion, and ability to handle sensitive situations.
