Home Care Field Registered Nurse (RN)

At Home Care Partners, a Licensed Home Care Agency, is seeking a dedicated and compassionate Home Care Field Registered Nurse (RN) to provide high-quality nursing care to clients throughout Delaware County, New York.

This full-time opportunity is ideal for an RN who enjoys working independently, providing one-on-one patient care, and supporting individuals and families in their homes and communities.

As a Home Care Field RN, you will travel throughout Delaware County to conduct nursing assessments, complete aide supervisions, develop Plans of Care, and help ensure each client receives safe, personalized, and respectful care.

Key Responsibilities

• Travel to client homes and community-based settings throughout Delaware County
• Conduct comprehensive in-home and community-based nursing assessments
• Perform head-to-toe evaluations, including medication and diagnosis reviews
• Complete aide supervisions and provide clinical guidance to caregivers
• Develop, review, and update individualized Plans of Care (POC)
• Orient aides to each client's Plan of Care and individual care needs
• Monitor clients for changes in condition and report concerns appropriately
• Communicate with patients, families, caregivers, aides, and office staff
• Complete accurate and timely clinical documentation
• Help ensure services are delivered safely and according to each client's needs
• Maintain compliance with applicable healthcare regulations and company policies

Registered Nurse Requirements

• Current and active New York State Registered Nurse (RN) license
• Ability and willingness to travel throughout Delaware County and assigned service areas
• Medical Professional Liability insurance
• Strong clinical assessment and documentation skills
• Comfortable using Gmail and Microsoft Word/Excel
• Excellent communication and interpersonal skills
• Ability to work independently in a field-based role
• Professional, dependable, organized, and compassionate
• Must meet all company hiring and onboarding requirements

Preferred Qualifications

• Previous home care, home health, outpatient nursing, or community health experience
• Familiarity with Medicaid, Medicare, or MLTC plans
• Experience conducting patient assessments and aide supervisions
• Experience developing, reviewing, or updating Plans of Care
• Strong clinical assessment and care coordination skills
• Bilingual in Spanish and English
• Comfortable working with patients from diverse backgrounds and with varying healthcare needs

Compensation & Benefits

Salary: $85,000 per year
