Coverage does not extend over the bridge. Physical Therapist Responsibilities

• Evaluate and treat patients in the home health setting
• Develop individualized treatment plans based on patient needs and goals
• Monitor patient progress and adjust care plans as appropriate
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Complete timely and accurate clinical documentation
• Educate patients and caregivers on therapeutic techniques, home exercise programs, and safety measures

Qualifications

• Graduate of an APTA-accredited Physical Therapy program
• Active Rhode Island Physical Therapist license
• Minimum of one year of clinical experience preferred
• Home health experience preferred but not required
• Valid driver's license, reliable transportation, and current auto insurance
• Ability to travel throughout the assigned territory
