LPN - Clinical Care Coordinator II

Clinical Care Coordinator II Provides patient care tailored to age‑specific needs and assists in managing patient visits under the supervision of a provider. Responsibilities Daily collaboration with physicians at the outpatient internal medicine office located in Waldemere Medical Plaza on the hospital campus. Qualifications BLS/CPR certification within 30 days of hire and graduation from an accredited nursing or diploma program. Florida Licensed Practical Nurse licensure. Schedule Monday through Friday with no weekends or holidays. Background screening through the Florida Care Provider Background Screening Clearinghouse mandatory for all care providers. Preferred Qualifications At least one year of outpatient clinical experience. Familiarity with electronic health records and practice management software.
