Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: July 13, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour (08:00 - 16:30), every other weekend required. Patient ratios: 1:25 weekdays, 1:45 weekends. 13-week assignment with 40 hours of orientation.

Requirements:

• Active California RN license (pending accepted if licensed by start date)
• Minimum 2 years acute care case management or discharge planning experience required
• Previous travel nursing experience required – first-time travelers not accepted
• Hospital experience mandatory; behavioral health experience does not substitute
• Epic charting system experience required
• Demonstrated competency in care coordination, utilization review, and discharge planning
• Experience arranging DME, SNF placement, and ambulance transport independently
• Knowledge of HMOs, IPAs, protective services, advance directives, and POLST
• Ability to organize and participate in family meetings and treatment plan development
• Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Two professional references covering one year within the last three years
