**Exciting Opportunity for an Acute Care Case Manager!** **Job Description:** - **Position**: Acute Care Case Manager - **Unit**: SMMC Case Management - **Profession**: Registered Nurse - **Specialty**: Case Management - **Shift**: Day 5x8-Hour (08:00 - 16:30) - **Years of Experience**: 2 years (Acute Care Case Management/Discharge Planning) - **Patient Ratio Experience**: - Days: 1:25 - Weekends: 1:45 - **Charting System**: Epic (required) - **Travel Experience**: Yes (seasoned traveler preferred) **Key Responsibilities:** - Oversee care coordination, discharge planning, and utilization review. - Conduct patient assessments and manage admission criteria. - Organize and participate in family meetings. - Develop and implement treatment plans. - Collaborate with interdisciplinary teams: Physical Therapy, Social Services, etc. - Arrange DME, SNF, and ambulance transport independently. - Navigate complex patient scenarios including drug/alcohol abuse, homelessness, mental health crises, and advance directives. **Requirements:** - Must have hospital experience; behavioral health experience does not apply. - Knowledge of CMS guidelines, DRGs, and evidence-based practices. - Ability to handle a daily case load of up to 25 patients. - No previous extensive travel experience needed; seeking seasoned travelers. - California state license required (pending license accepted if licensed upon start date). - Limit of 1 Pre-Approved Time Off Request; maximum of 7 days RTO restrictions. - Orientation hours: 40 **Why Catalytic Solutions (CatSol)**: At CatSol, we connect dedicated therapists with rewarding assignments across the country.
